COVID VACCINATION INFORMATION

UPDATE
National Health Service procedures, regarding vaccination against COVID-19 of the public treated by the private health sector, have now been updated leading to changes in our own internal protocols.
Anyone meeting the inclusion criteria for Phase 1 of the COVID Vaccination Program will have to attend a presential medical consultation to ensure that the correct information is entered onto the electronic platform.
See the Phase 1 vaccination criteria in the list below.
We are awaiting access to the above-mentioned computer platform which will then enable us to comply with this requirement and therefore book appointments requested by our clients.

NOTE:
  • Before attending a consultation, it is necessary for the client to have a National Health User Number (if not, they are required to contact or go to their local Health Centre to obtain this).
  • We still do not have information on what the procedures will be for Phase 2 and 3 of the Vaccination Program, however the medical evaluation of your clinical history can be done beforehand.

  • COVID VACCINATION INFORMATION
Concerning LUZDOC clients who have already been given a medical declaration:

National Health Service procedures, regarding vaccination against COVID-19 of the public who are monitored by the private health sector, have now been updated leading to a review and immediate changes in the Luzdoc internal protocols.
According to a communique from the NHS, the attending private sector physician must complete an electronic declaration on a public information sharing platform.
Therefore, all persons who have already requested clinical information from us and to whom the resulting declaration has been delivered, must schedule a medical consultation to finalise / carry out this procedure in accordance with the new regulations.

Providing this electronic declaration will guarantee your access to the vaccination program, leading to subsequent contact by your Health Centre for scheduling.
If you have already delivered the medical declaration to your Health Centre, we cannot guarantee that the process will be completed as, according to the official information we have received, this can only be guaranteed by filling in the necessary data on the electronic declaration platform.
If you do not wish to attend a medical consultation at Luzdoc, we cannot be made responsible for the fact that the previous information may not be sufficient to guarantee the completion of the process.
